Tracy Canonizado, DNP

Tracy Canonizado, DNP, is a certified family nurse practitioner (FNP) providing primary care through Perlman Clinic, an independent practice with clinic offices throughout San Diego. She is a member of the UC San Diego Health Physician Network.

UC San Diego Health Physician Network is an alliance of health care providers based in San Diego, Riverside and Imperial counties. Members of the network collaborate on developing shared systems, infrastructure and quality initiatives to provide high-value health care in the region.

As a family nurse practitioner, Ms. Canonizado takes care of the primary and urgent care needs of people of all ages. She is passionate about health care and educating patients on health issues, both chronic and acute. She strives to build long-lasting clinician-patient relationships and work together to develop wellness plans. 

Ms. Canonizado earned her doctorate degree in nursing practice from University of Hawaii at Manoa in Honolulu, Hawaii. She is certified by the American Nurses Credentialing Center and a member of Sigma Theta Tau International Honor Society of Nursing. Ms. Canonizado is fluent in Cantonese.

Outside of work, Ms. Canonizado enjoys spending time with her husband, son, and their dog Crimson, a Maltese/Bichon mix. Her favorite activities with them are the simple ones: playing in the park, brunch at home and having family over for barbecues in the backyard.
